B.02 Recessive mutations in ATP8A2 cause severe hypotonia, cognitive impairment, hyperkinetic movement disorders and progressive optic atrophy

Background:ATP8A2 mutations have only recently been associated with human disease. We present the clinical features from the largest cohort of patients with this disorder reported to date. Methods: An observational study of 9 unreported and 2 previously reported patients with biallelic ATP8A2 mutations was carried out at multiple centres. Results: The mean age of the cohort was 9.4 years old (range: 2.5-28 yrs). All patients demonstrated developmental delay, severe hypotonia and movement disorders: chorea/choreoathetosis (100%), dystonia (27%) or facial dyskinesia (18%). Hypotonia was apparent at birth (70%) or before 6 months old (100%). Optic atrophy was observed in 75% of patients who had a funduscopic examination. MRI of the brain was normal for most patients with a small proportion showing mild cortical atrophy (30%), delayed myelination (20%) and/or hypoplastic optic nerves (20%). Epilepsy was seen in two older patients. Conclusions:ATP8A2 gene mutations have emerged as a cause of a novel phenotype characterized by developmental delay, severe hypotonia and hyperkinetic movement disorders. Optic atrophy is common and may only become apparent in the first few years of life, necessitating repeat ophthalmologic evaluation. Early recognition of the cardinal features of this condition will facilitate diagnosis of this disorder.

Objective.
Individuals who seek asylum are frequently fleeing violent persecution and may experience head injury (HI). However, little is known about the prevalence of HI in asylum seekers and refugees (ASR) despite the potential for HI to significantly affect cognitive and emotional functioning and to compromise asylum outcomes. This preliminary study investigates the prevalence of HI in ASR referred to a complex psychological trauma service.
Method.Participants were 115 adult ASR referred to a community psychological trauma service with moderate to severe mental health problems associated with psychological trauma. They were screened for a history of HI using a questionnaire developed for the study. Interpreters were used when required.
Results.The overall prevalence of HI was 51%. At least 38% of those with HI had a moderate–severe HI that could cause persisting disability. In 53% of those with HI, the cause was torture, human trafficking or domestic violence. Repeat HI can have cumulative effects on function; it was common, and was reported in 68% of those with HI. An injury to the head was not known to mental health clinicians prior to screening in 64% of cases.
Conclusion.The emotional and cognitive consequences of HI in ASR may increase the vulnerability of this disadvantaged group, and can be associated with neurobehavioural problems affecting daily life and may compromise asylum outcomes. Routine screening for HI in ASR is needed, as are links to neuropsychology and brain injury services for advice, assessment and intervention.

Summary
Head injury is the leading cause of disability and death in young adults. The main causes are accidents and assaults, and in a significant proportion of cases alcohol or drugs are involved. Head injury can have significant effects on cognition, personality and behaviour. Damage to brain areas that have been associated with antisocial behaviour is common after head injury. Post-traumatic amnesia refers to a period of time from recovery of consciousness until return of orientation and continuous memory for day-to-day events. It is relevant to consider the heterogeneity within the head injury population both in terms of severity of injury and outcome. Sexual offending and violence are significant problems after head injury, particularly in males, although they are limited by their retrospective non-blind design. Where there is a distant history of a head injury, the occurrence and severity should be verified in medical records.

The Value of Fabry-Perot Interferometry in Studying Long-Term Convective Line Shifts

Small < 10 m s−1 variations of radial velocity (RV) with multi-year periods in solar-type stars may be indistinguishable from the effects induced on lines by stellar activity cycles (Dravins 1985; Saar & Donahue 1997). Dravins (1992) recommended a resolving power R > 3 × 105 to measure accurately the subtle changes in the shapes of bisectors of photospheric absorption lines driven by changes of granular convection in slowly rotating dwarf stars. Butler et al. (1996) measure impressively small amplitudes of RVs by using echelle spectrographs that cover a broad spectrum. However, to cover a broad spectrum the resolving power is typically limited to < 7 × 104, and the necessary presence of the iodine absorption spectrum may make it difficult to measure convective line shifts contemporaneously with the RV time series. Furthermore, to reach an RV accuracy of ± 3 ms−1 the whole profile of each line is used, thus maximizing the possibility that changes in the shapes of the lines’ C-bisectors could induce an apparent variation of RV.
Dravins (1985) recommended the exclusive use of the steep flanks of photospheric absorption lines to minimize the effects of convection on apparent RV. McMillan et al. (1993, 1994) demonstrated that such RV measurements made with a Fabry-Perot etalon (FPE) interferometer in transmission can be stable in the presence of stellar line variations seen by other investigators whose measurements were not based exclusively on line flanks. Dravins also prescribed high resolving power, high signal-to-noise ratio, high instrumental contrast, and low instrumental wings to analyze the rest of the line profile for convectively-driven changes (Dravins 1978, 1987, 1992). A double- or multiple-pass FPE scanning whole line profiles can provide high R, high contrast, low wings, and a stable, symmetrical line spread function with small (portable) optics, although the low photon efficiency will restrict its use to a few carefully selected stars. The spectral classes of these stars should span the spectral classes of the stars being monitored by others for planets. We describe a possible implementation of this concept that has the potential for adequate photon flux: observing symbiotically through another instrument on a 6-m to 10-m class telescope.

Post-traumatic stress disorder (PTSD) is described following a road traffic accident in which the patient suffered a severe head injury. The stress reaction was associated with intrusive thoughts and avoidance of cognitive and physical events associated with consequences of the accident. The condition was successfuly treated by behavioural intervention. It seems clear that PTSD can occur even where there is loss of consciousness and organic amnesia for the event and its immediate sequelae.
